چکیده

Methods This is a prospective observational study with institutional review board approval. Patients TBI and a Glasgow Coma Scale score ≤8 were identified on admission. Glasgow Coma Scale (GCS), Injury Severity Score (ISS), radiographic scales based on admission computed tomography (Marshall), IntraCranial Pressure (ICP), pBtO2 and pBtO2/paO2 during first 24 hours were recorded. Patient outcome was determined as favorable (patient had no, little, or moderate disability) or unfavorable (patient died, was in a vegetative state or had severe disability) using the Glasgow Outcome Scale by medical record review at 6 months after TBI. We considered an ordinal analysis with proportional odds methodology. A logistic regression model was used to calculate the 95% CI for the odds of unfavorable outcome.

A Prospective Randomized Study of Brain Tissue Oxygen Pressure-Guided Management in Moderate and Severe Traumatic Brain Injury Patients

The purpose of this study was to compare the effect of PbtO2-guided therapy with traditional intracranial pressure- (ICP-) guided treatment on the management of cerebral variables, therapeutic interventions, survival rates, and neurological outcomes of moderate and severe traumatic brain injury (TBI) patients. From 2009 to 2010, TBI patients with a Glasgow coma scale <12 were recruited from 6 c...

Brain tissue partial pressure of oxygen predicts the outcome of severe traumatic brain injury under mild hypothermia treatment

OBJECTIVE The aim of this study was to investigate the clinical significance and changes of brain tissue partial pressure of oxygen (PbtO2) in the course of mild hypothermia treatment (MHT) for treating severe traumatic brain injury (sTBI). METHODS There were 68 cases with sTBI undergoing MHT. PbtO2, intracranial pressure (ICP), jugular venous oxygen saturation (SjvO2), and cerebral perfusion...

Patient transport and brain oxygen in comatose patients.

OBJECTIVE Transport of critically ill intensive care unit patients may be hazardous. We examined whether brain oxygen (brain tissue oxygen partial pressure [PbtO2]) is influenced by transport to and from a follow-up head computed tomography (transport head computed tomography [tHCT]) scan. METHODS Forty-five patients (24 men, 21 women; Glasgow Coma Scale score < or =8; mean age, 47.3 +/- 19.0...

The relationship between intracranial pressure and brain oxygenation in children with severe traumatic brain injury.

BACKGROUND Intracranial pressure (ICP) monitoring is a cornerstone of care for severe traumatic brain injury (TBI). Management of ICP can help ensure adequate cerebral blood flow and oxygenation. However, studies indicate that brain hypoxia may occur despite normal ICP and the relationship between ICP and brain oxygenation is poorly defined. This is particularly important for children in whom l...

Reduced brain tissue oxygen in traumatic brain injury: are most commonly used interventions successful?

BACKGROUND Brain tissue oxygenation (PbtO2)-guided management facilitates treatment of reduced PbtO2 episodes potentially conferring survival and outcome advantages in severe traumatic brain injury (TBI). To date, the nature and effectiveness of commonly used interventions in correcting compromised PbtO2 in TBI remains unclear. We sought to identify the most common interventions used in episode...
